Insurances accepted are Aetna, Affinity, Alicare, Amerigroup, Cigna, Empire Blue Cross, Healthnet, Local 1199, Medicare, Oxford, Unicare and United Healthcare.
The clinical office will assist you with appointments, insurance, directions, etc. Please bring copies of previous medical records including test results, biopsy and endoscopy reports—do not forward your medical records in advance of your visit—bring them with you!
Payment is expected at the time of consultation. Credit cards are not accepted.

Nutritionist
Anne Lee, EdD, RD, LD
A one hour consultation with Dr. Anne Lee, the Center’s nutritionist, is $200. There is no charge for patients who are seen by physicians in the Celiac Disease Center at Columbia Universty. Insurances are not accepted. Payment is expected at the time of consultation. Credit cards are not accepted.
In preparation for your consultation with the nutritionist, please keep a food log 3 days prior to your visit. In addition, if you are taking vitamins or supplements, please bring them with you in the original container.
When the physician participates in the plan, the patient is required to pay a co-pay and in most instances (not all) provide a referral form on the day of the visit. Please note that not all plans require a referral and the co-pay varies based on your specific plan. Please check with your insurance company.
When the physician does not participate in the plan, full payment is expected on the day of the visit. The Center’s clinical office will submit a claim to your insurance company following your visit and you will be reimbursed whatever you are entitled to based on your specific insurance plan after you meet your deductible and co-insurance. Please check with your insurance company for further clarification.
